Health at the Hub

Patients can now access a range of Health and Wellbeing services at the Melbourn Hub via Meridian Primary Care Network's Personalised Care Team. Services include help with anxiety, pressure of being a carer, giving up smoking, healthier eating and the menopause, amongst many other aspects of general wellbeing.

The initiative, is a partnership between Melbourn Hub and Meridian Primary Care Network (PCN) and is the first of its kind in South Cambs. The idea is to bring some of the above services out of GP surgeries and into a community setting, to provide easier access for patients. Members of the Meridian Personalised Care Team including Social Prescribers, Health Coaches and Care Co-ordinators based at Melbourn Hub, will be able to offer help to patients who are seeking certain mental and physical health advice and support.

GP’s, health practitioners and community support services came together at Melbourn Hub on Saturday 25th June at an event to launch ‘Health at the Hub’ to the public.

GP’s from Melbourn, Bourn and Roysia surgeries were on hand to talk to the public, along with the Personalised Care Team covering the six GP surgeries that make up Meridian PCN. Some of the organisations at Saturday’s launch event included Mind CPSL, South Cambs District Council Health Services including ‘Talking Communities’, Homestart, Mobile Warden Scheme, Community Support for Dementia, Community Transport and Age UK.

‘Healthy You’ provided free health checks, the NHS Covid vaccination bus promoted their tour service and The Hub’s library had a range of health and well-being books on show.

The event was very well attended by the public who were extremely positive about the new initiative and the service providers themselves had an opportunity to network and share support for each other.     

Amongst those visiting the event were South Cambs District Council leader Bridget Smith, Bill Handley South Cambs lead cabinet member for Communities and Jan Thomas, CEO for Cambridgeshire & Peterborough Integrated Care System who said “Thank you and congratulations to all who have worked so hard on Melbourn ‘Health at The Hub’. It’s a great example of a community space designed for local people by local people. We look forward to learning from the team and ensuring we can support others to create similar excellent local support.”

Meridian PCN members of the Personalised Care Team will be now be at Melbourn Hub to begin offering their services and also hold a ‘walk in’ advice/information session open to all, every Monday between 10am and 11.30am, for patients to find out how they can access the services they need from The Hub.
